Start With the State Profile

Look for Patterns, Not a Single Scary Number
One historical deficiency does not automatically tell you the current quality of a community. Repeated or recent patterns deserve more attention and should generate specific tour questions.
Ask the Facility to Explain
If a state record raises a concern, ask what happened, what corrective action occurred and how the process changed. Do not rely on a salesperson's dismissal without reviewing the underlying record.
Combine Regulatory and Care-Fit Questions
A clean-looking record does not prove the community is right for a particular resident. Staffing, care scope, medication procedures, emergency planning and total pricing still require direct evaluation.
